Here's how Dana works with HMO patients: You self-pay for your new patient visit, and Dana creates a personalized treatment plan your primary care doctor can implement. Dana stays available to support your doctor along the way, so you're not paying for follow-up visits you don't need.

We can provide a superbill after your visit that you can send to your insurance for possible out-of-network reimbursement.

What's a Superbill?

A Superbill is an itemized receipt with the medical codes your insurance needs to process an out-of-network claim.
Reimbursement amounts vary by plan, so check with your insurance about your out-of-network coverage before your visit.
Here's how it works:

After your appointment, we send you a Superbill.

You submit the Superbill to your insurance.
